I hereby apply for an Massachusetts Chapter of the Appraisal Institute Scholarship for one Appraisal Institute course qualifying me for a designation. I represent to the best of my knowledge and belief, that all information contained on this application is true and accurate as of the date of this application. Should any such information change after the date of this application, I will immediately notify the Chapter Scholarship Committee. I understand that if any information on this application is false, I will be ineligible for a scholarship; I must return any scholarship monies awarded to me; and I may be subject to discipline pursuant to the Appraisal Institute's Bylaws and Regulations. I hereby irrevocably waive any claim or cause of action at law or equity that I might have at any time against the Appraisal Institute
